By accessing our website, scheduling a complimentary call, purchasing a service, or participating in our coaching or educational services, you agree to these Terms & Conditions.

If you do not agree with these terms, please do not use our website or services.

 

1. Our Services

My Surgical Wellness provides nurse-led wellness coaching, evidence-based education, surgical readiness support, patient advocacy, and lifestyle guidance.

Services may include support related to:

    ●    Preparing for surgery

    ●    Understanding and organizing information provided by your healthcare team

    ●    Preparing questions for your healthcare providers

    ●    Recovery planning and support

    ●    Nutrition and lifestyle habits

    ●    Sleep, stress, and overall wellness​ and general wellness goals

    ●    Medication, supplement, and lab pattern review

    ●    GLP-1 lifestyle and wellness support

    ●    Other wellness and health-education topics within the scope of our services

Our services are intended to complement, not replace, the care and instructions provided by your physician, surgeon, prescribing provider, or other licensed healthcare professional.

 

2. Not Medical Care

My Surgical Wellness provides coaching, education, guidance, support, and patient advocacy.

Our services do not constitute medical diagnosis, medical treatment, emergency care, or a replacement for your healthcare provider.

My Surgical Wellness does not:

    ●    Diagnose medical conditions

    ●    Treat disease

    ●    Prescribe medications

    ●    Change or direct medication doses

    ●    Discontinue medications on your behalf

    ●    Replace your physician, surgeon, or prescribing provider

    ●    Provide emergency medical care

    ●    Make surgical decisions for you

    ●    Provide individualized medical treatment plans

Information provided during coaching sessions is intended to help you better understand and implement the plan and recommendations provided by your healthcare team.

You remain responsible for discussing medical questions, symptoms, medications, supplements, surgical decisions, and changes in treatment with your appropriate healthcare provider.

3. Your Healthcare Provider Remains Your Medical Resource

You should continue to work directly with your physician, surgeon, prescribing provider, and other members of your healthcare team for medical care.

My Surgical Wellness does not override, replace, or supersede instructions provided by your healthcare professionals.

If information discussed during a coaching session differs from instructions provided by your healthcare provider, follow your healthcare provider’s instructions and contact them for clarification.

 

4. Emergency Situations

My Surgical Wellness is not an emergency or urgent-care service.

If you are experiencing a medical emergency, severe symptoms, or a potentially life-threatening situation, call 911 or seek immediate medical attention.

Do not use email, text messaging, website forms, coaching sessions, or other My Surgical Wellness communications for emergency medical concerns.

Limits of Confidentiality

My Surgical Wellness respects your privacy and keeps the information you share confidential, with important exceptions. As a licensed nurse, the coach is required by law to report certain information to the appropriate authorities, and may disclose information without your consent, when:

• There is reasonable suspicion of abuse, neglect, or exploitation of a child, an at-risk adult, or an elder

• You express intent or thoughts of harming yourself, and there is concern for your immediate safety

• You express intent to harm another person

• Disclosure is otherwise required by law, court order, or professional licensing obligations

In these situations, we may contact emergency services, law enforcement, protective services agencies, or other appropriate parties. We will share only the information reasonably necessary to address the situation.

My Surgical Wellness is not a crisis or emergency service. If you are experiencing a medical or mental health emergency, call 911. For thoughts of suicide or self-harm, call or text 988 (Suicide & Crisis Lifeline).

 

5. Client Responsibilities

By participating in our services, you agree to:

    ●    Provide information that is accurate and complete to the best of your knowledge

    ●    Communicate openly about your goals and concerns

    ●    Follow the instructions and recommendations of your healthcare providers

    ●    Ask your healthcare provider when medical clarification is needed

    ●    Notify us of relevant changes that may affect your coaching experience

    ●    Use our services for their intended educational and coaching purposes

    ●    Maintain appropriate communication and behavior during interactions with My Surgical Wellness

You are ultimately responsible for your own healthcare decisions and actions.

 

6. No Guarantee of Results

Wellness and lifestyle outcomes vary from person to person.

My Surgical Wellness does not guarantee any particular health, wellness, surgical, recovery, weight, metabolic, or other outcome. Our role is to provide education, support, accountability, and guidance to help you work toward your personally identified goals.

 

7. Complimentary Calls

Complimentary calls are intended to help determine whether My Surgical Wellness services may be appropriate for your needs.

A complimentary call does not establish a medical provider-patient relationship and does not constitute medical advice, diagnosis, or treatment.

 

8. Scheduling and Appointments

Appointments are scheduled according to availability. Please provide accurate contact information when scheduling. If you need to cancel or reschedule an appointment, please provide 24 hour notice. Specific cancellation, rescheduling, or no-show policies may apply to individual services or packages and will be communicated at the time of purchase or scheduling.

 

9. Payments

Services must be paid for according to the pricing and payment terms presented at the time of purchase.

Payment may be processed through third-party payment providers. By submitting payment, you authorize the applicable payment provider to process the transaction according to its terms and policies. All prices are subject to change. Pricing changes will not affect services that have already been purchased unless otherwise stated.

 

10. Packages and Unused Sessions

Some services may be offered as packages consisting of multiple coaching sessions. Package sessions are intended to be used within the timeframe communicated at the time of purchase. Unless otherwise stated in the service description or agreed upon in writing, unused sessions may expire after the applicable package period. If circumstances require an adjustment to a package schedule, please contact My Surgical Wellness.

 

11. Refunds

Refund policies may vary depending on the service or package purchased. Because coaching sessions involve reserved appointment time and preparation, completed sessions are not refundable. If you have a question or concern regarding a purchase, please contact My Surgical Wellness so that the situation can be reviewed. Nothing in this section limits any rights you may have under applicable law.

 

12. Communication

You may communicate with My Surgical Wellness through email, telephone, scheduling platforms, client-management platforms, forms, or other communication methods we make available. Electronic communications may not be completely secure. Please avoid sending highly sensitive information through ordinary email, website contact forms, or text messages unless specifically requested through an appropriate secure process. My Surgical Wellness will make reasonable efforts to respond to communications in a timely manner, but communication channels are not monitored continuously.
